Job Description
The Care Coordinator provides care coordination, case management, and linkage for Thresholds members receiving care at Patient Centered Care clinics, to resources designed to improve social determinants of health. The role assists in implementing program improvements and developing data tracking and reporting.
Requirements
- Collaborate with Thresholds Community Support teams and Patient Centered Care (PCC) Wellness Clinics
- Assist with navigating systems and maintaining entitlements including Social Security Income (SSI) and Medicaid
- Develop tracking system to help teams manage redetermination dates, documentation, and appointments
- Collaborate with Program Director, Vice President, Clinical Operations Direct or Integrated Partnerships, and Senior Vice President, Clinical Operations
