Job Description
Transitions Hospice is seeking a Certified Nursing Assistant (CNA) to provide dignified care to patients in their homes. As a vital member of the interdisciplinary care team, you will provide personal care with dignity and respect, attend to patient needs promptly, and communicate with patients and families in a positive and compassionate manner.
Requirements
- Active CNA License or Certification in the state of employment
- Possess and maintains current CPR certification
- One (1) + year Hospice, home health or related organization experience preferred
- Comfort and sensitivity working with terminally ill patients and their families
- A calm, dependable presence under emotionally demanding circumstances
- Strong attention to detail in observation and documentation
- A valid driver's license and willingness to travel locally for patient visits
