Job Description
Transitions Care is a leading hospice organization that provides 24/7 access to services in the comfort of the patient's home, offering physical, emotional, and spiritual support. As an Advanced Nurse Practitioner, you will be responsible for the direction and oversight of care and treatment for hospice patients and provide oversight for Plan of Care and all changes.
Requirements
- Consult and work with the patient's attending physician
- Assure compliance with local, state, and federal laws, Medicare regulations, and HIPAA
- Implement the plan of care to meet the patient’s medical needs, assess and reevaluate the patient’s medical needs on a regular basis, and manage related expenditures in a fiscally responsible manner
