As a Transitions Care RN Case Manager, you will plan, organize, and direct hospice care for patients and families in their homes and communities.
Requirements
- Graduate of an accredited school of Nursing
- Registered Nurse licensed in the State
- One (1) + year Hospice, home health or related organization experience preferred
