Job Description
Transitions Care is a leading hospice organization providing 24/7 access to services in the comfort of the patient's home. As a Hospice Nurse (RN), you will provide care to patients in need and work as a support team member to the IDG team.
Requirements
- Provides professional nursing care by utilizing all elements of the nursing process within the designated scope of practice.
- Assesses and evaluates patient's status by writing and initiating a plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care.
- Administers medications and treatments as prescribed by the physician in the physician's plan of care.
