Job Description
Provide professional nursing care to patients in need, assessing and evaluating patient's status, and completing visit types to meet patient and family needs. Utilize health assessment data to determine nursing diagnosis and administer medications as prescribed. Communicate with physicians and community health related persons to coordinate care plans.
Requirements
- Graduate of an accredited school of Nursing
- Nursing licensure in the State which you provide care (RN)
- One (1) + year Hospice, home health or related organization experience preferred
