Hospice Nurse (RN or LPN) – Weekend Shift / Part-Time

Transitions Care is a leading hospice organization that provides 24/7 access to services in the comfort of the patient's home. As a Hospice Nurse, you will provide care to patients in need and support the IDG team.

Requirements

• Provides professional nursing care by utilizing all elements of the nursing process within the designated scope of practice.
• Assesses and evaluates patient's status by writing and initiating a plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care.
• Administers medications and treatments as prescribed by the physician in the physician's plan of care.
• Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required.
• Fulfills obligation of requested and/or accepted case assignments.
