Job Description
Transitions Hospice is seeking an RN Case Manager to provide compassionate and unhurried care to patients and their families in their homes, coordinating care, guiding interdisciplinary teams, and focusing on the moments that matter most.
Requirements
- Active RN license in the state of employment
- Graduate of an accredited school of Nursing.
- One (1) + year Hospice, home health or related organization experience preferred.
- Compassion for patients and families navigating end-of-life care
- Strong clinical judgment and the ability to work independently in the field
- Excellent communication skills across care teams and families
