RN Care Manager - Hybrid/Remote

RN Care Manager position focusing on ambulatory care for Trinity Health medical group patients with multiple chronic diseases, CHF, and COPD. The position will involve disease education, social influencers of health mitigation, service coordination, and provider office collaboration to ensure integrated care and improved health outcomes.

Requirements

• Associates Degree required, BSN preferred
• Previous experience in care management in disease management, home health care, hospital, or physician office setting
• Certification in Case Management (CCM) preferred
• 5 years of RN experience
• Prior electronic medical record (EMR) experience required
• Previous project leadership experience required
• General knowledge of insurance plans and clinical resource management
