RN Case Manager Home Health, Sign on Bonus Eligible!

We are seeking a Registered Nurse Case Manager to join our Home Health team in Malden, MA. The successful candidate will manage and deliver comprehensive home health services to clients, including assessments, interventions, and supportive care. The position requires excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Requirements

• Massachusetts/New Hampshire RN Licensure
• Valid state issued Driver's License
• One (1) year of acute medical/surgical nursing experience
• Cardiopulmonary Resuscitation (CPR) Certification
