Job Description
The RN Case Manager serves as case manager for the interdisciplinary team (IDT) and is responsible for the delivery of quality hospice nursing care for each patient assigned.
Requirements
- Massachusetts RN Licensure
- New Hampshire RN Licensure
- Current CPR Certification
- One (1) year of hospice and home health experience
