Job Description
The Social Worker - Case Manager will assess, plan, and coordinate care for patients from admission to discharge, ensuring adherence to the Case Management Model. The position requires a Master's degree in Social Work, LICSW/LCSW licensure, and 3 years of Social Work experience.
Requirements
- Master’s degree in Social Work
- Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Social Worker (LCSW)
- Three (3) years of Social Work experience
- Current Basic Life Support (BLS) Certification
