Part-Time Registered Nurse

The Care Manager will manage a small caseload of private pay clients, provide coordination of services, resources and ongoing monitoring and management to clients in their home. This includes developing strong relationships with Community and professional organizations to generate client referrals.

Requirements

• Registered nurse with 7-10 years of experience working in Geriatrics
• Ability to work with minimal supervision
• Valid Driver’s License and dependable personal vehicle
