Care Coordinator - Youth

Join Unison Health to help children and youth with behavioral or developmental health challenges and their families. Collaborate with children, families, and community partners to connect youth to care and services. Work in a hybrid model of remote/office and in-person visits. Participate in training and professional development opportunities.

Requirements

• High School Diploma with 3 years' experience
• Associate or Bachelor's degree with 2 years' experience
• Master's degree with 1 year experience
• Experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field
• Expertise in family systems, community systems/resources, case management, child and family counseling or therapy, child protection, or child development
• Proficient in computer systems and software
• Valid driver's license, reliable transportation, and be insurable under the agency's commercial policy
• LSW/LPC licensure preferred
