The Case Management Coordinator position is responsible for coordinating patient progress through the system, collaborating with physicians and clinical staff, and ensuring compliance with regulatory standards. The role requires creative problem-solving and excellent communication skills to provide patient-centered care.
Requirements
- Registered Nurse with a license to practice in the State of New Jersey required
- Bachelor's Degree in Nursing or a health care related field required
- Minimum of four (4) years of acute care hospital clinical experience required
- Case management certification from either CCM, ACMA or RN-BC preferred
- Basic computer skills required
- Ability to work effectively and collaboratively with interdisciplinary teams
