Supportive Services Coordinator

The Supportive Services Coordinator will hold a caseload that is assigned through internal referrals from our testing and research projects, managing retention in care for their caseload and following fidelity monitoring protocols. They are expected to actively pursue required knowledge and skills for professional development and stay informed about the resources available on the south side of Chicago.

Requirements

• Calls community members and conducts resource counseling, completing all related documentation and notes
• Researches appropriate and individualized resources for community members, specifically resources for people of color and those who identify as LGBQ and TGNC
• Works with community members over the phone or in-person
• Assists with day-to-day Supportive Services tasks
• Provides HIV/STI treatment, education, and prevention in non-traditional settings
• Performs phlebotomy or willing to be trained in phlebotomy
• Supports evening and weekend events where Supportive Services is requested
• Assists with and performs various administrative and operational tasks under direct supervision
