Job Description
Case Manager for the Innovative Urban Village supportive housing program in New York, providing comprehensive, trauma-informed, and culturally responsive supportive services to families impacted by domestic violence. Key responsibilities include conducting assessments, developing individualized service plans, providing counseling and crisis intervention, and coordinating referrals to promote long-term housing stability, safety, and independence.
Requirements
- Bachelor's degree in Social Work or related field
- Minimum of 2 years of experience in case management or direct service
- Knowledge of OTDA regulations, ESSHI guidelines, and supportive housing best practices
- Strong crisis intervention, de-escalation, and safety planning skills
- Ability to work with diverse populations including LGBTQIA+ individuals, immigrants, older adults, and individuals with mental health or substance use
