Licensed Professional Counselor

Licensed Professional Counselor responsible for evaluating individual and family needs after receiving a diagnosis and creating a custom treatment plan. Provides individual, couples and family therapy to improve mental health and coping skills.

Requirements

• Evaluates individuals and family's needs after receiving diagnosis and create a custom treatment plan
• Provides individual, couples and family therapy to improve mental health and coping skills
• Facilitates and leads various therapeutic and support groups for patients and caregivers
• Develops innovative support and community engaging programs for caregivers and patients
• Coordinates with other Providers, agencies and community resources to provide comprehensive care
• Connects families and caregivers to institute and community resources needed to improve quality of life
• Performs all other duties as assigned
