Job Description
Integrated Behavioral Health Provider to provide holistic, preventive, therapeutic, and educational services for patients of all ages in a rural community, promoting patient and family-centered care.
Requirements
- Perform assessments, identify problems, justify medical necessity, and deliver evidence-based treatments, crisis intervention, and individual/family/group therapy.
- Work within the Primary Care Behavioral Health (PCBH) model to develop and implement treatment plans through consultation, brief assessments, and psychoeducation.
- Maintain accurate and up-to-date patient records in our EMR system (NextGen), including intake documentation and treatment plans.
