Licensed Practical Nurse-Choice Care Coordinator

Valor Healthcare is looking for a Licensed Practical Nurse to join their team at the Community Based Outpatient Clinic (CBOC) in Durango. The LPN will provide patient-centered care coordination, education, and follow-up services to support veterans.

Requirements

• Diploma or certificate of an accredited program of practical or vocational nursing
• Minimum one-year experience as an LPN/LVN in a related primary care or ambulatory care setting
• Credentialed and in good standing through the Veterans Health Administration (VA)
• Current certification in Basic Life Support and ACLS
• Proficiency in written and spoken English
• Strong computer skills, including EMR experience
