As a Physical Therapist, you’ll provide exceptional services within Assisted Living, Skilled Nursing, and Memory Care residents. This role will foster a positive, supportive environment for residents, families, and team members on a PRN basis.
Requirements
- Responsible for the maintenance and accuracy of records in regard to patient care for all patients on personal case load.
- Provide direct patient care as prescribed, including evaluation and treatment planning.
- Perform clinical functions: screens, evaluations, development of treatment plans, and delivery of treatment in accordance with facility, Medicare, and Facility’s policies and procedures.
