Records physical findings, formulates plan and prognosis, and prescribes treatment. Collaborates with healthcare professionals to prepare patient care plans. Monitors patient progress and reports to supervising physician.

Requirements

• 2+ years experience in hospice, home health, or community health
• Completion of accredited Nurse Practitioner program
• State licensure
