Candidate must have experience in Assisted Living Regulations and management. Must possess a valid LPN or RN license, in good standing in accordance with laws of this state, from an approved State School of Nursing. A minimum of one (1) year of full-time or equivalent clinical experience is required.

- In exigent circumstances, the employee may be subject to call back during emergency conditions (e.g., severe weather, evacuation, temporary staffing shortages, etc.).

- Responsible for the retention of residents in the community.
- Ensures provision of services to the residents and ensures that all Federal and State policies governing resident care are closely monitored and carried out.
- Ensure all Viva Senior Living clinical policies are followed, including assessments, service plans, resident care, medication management, and infection control.
- Interview, hire, evaluate, motivate, and monitor the performance of the care staff in the community.
- Ensure the completion of assessments on all residents, as required by Viva Senior Living policies and state requirements.
- Coordinate departmental schedule and ensure adequate staffing in accordance with company standards, policies, procedures, budgets, and state law.
- Oversees training programs for direct care staff in the community, as regulated by Federal and State licensing requirements.
- Conducts orientation for new team members of the department to ensure that all employees are knowledgeable of the goals, policies, and procedures of the department.
- Conduct interviews/observations on a continual basis of residents to alert Executive Director, family, and other appropriate individuals of any changes in the physical or emotional health of the resident.
- Supports administration in referral development and maintaining a high census, act as care manager and resource person for the residents and their families.
- Measures outcomes, set goals and leads the care team to achieve those outcome goals
- Performs all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough handwashing, use of disposable gloves where indicated and proper disposal of soiled materials.
- Perform various tasks assigned by the charge nurse, including checking vital signs, weighing residents, applying creams/ointments, and collecting specimens.
- Ensures that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals’ needs and rights.
- Report any occupational exposures to blood, body fluids, infectious materials, and or hazardous chemicals in accordance with facility policy.
- Report work related injuries and illnesses immediately to your supervisor.
- As a condition of employment, complete all assigned training and skills competency.
- Participate in all life safety and emergency drills and trainings.
- Fulfill responsibilities as assigned during implementation or activation of the facility’s emergency plan.
- Assists with evacuation of residents in an emergency situation
- Maintain confidentiality of Protected Health Information, including verbal, written and electronic communications.
- Report non-compliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel.
- Protect residents from abuse and cooperate with all investigations.
- Report any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor and/or administrator.
- Report any retaliation or discrimination to the Human Resource Director and the Compliance Officer.
- Fulfill individual responsibility to report any suspicion of a crime to law enforcement and the state agency.
