Job Description
As an Independent Optometrist, you will launch your services in Sam's Club and take a leading role in patient care with respect to eye and vision care as well as general health and well-being. You will have autonomy to establish your days of practice, hours of operation, patient schedule, and fee schedule, and keep 100% of exam/professional fees.
Requirements
- Doctor of Optometry (OD) from an accredited optometry school
- Valid state optometry license
- General Liability Certificate
- Professional Liability Insurance Certificate
- W-9
- Ophthalmologist (MD or DO) from an accredited school
- Valid state ophthalmology license
