Social Worker

WelbeHealth is seeking a Social Worker to join their interdisciplinary team (IDT) to provide comprehensive primary care and social services for frail seniors. The Social Worker will maintain a caseload of participants, conducting psychosocial assessments, care planning, counseling, and case management. The role requires a Master's degree in social work (MSW) and a minimum of one year of experience working with a frail or elderly population.

Requirements

• Master’s degree in social work (MSW) required
• Minimum of one (1) year of experience working with a frail or elderly population preferred
• Experience coordinating and facilitating care conference meetings preferred
• Experience assisting people with behavioral health or substance abuse issues preferred
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
