Job Description
Nurse Practitioner (APP) delivers field-based care in the comfort of a patient’s home, managing chronic disease, collaborating across the continuum, and preventing ER visits.
Requirements
- Advanced Degree: Master’s Degree (NP) with a preferred focus on Adult, Geriatric, and/or Family.
- Ohio Credentials: Active, unencumbered NP license and Board Certification (ANCC or AANP).
- Reliable Mobility: Valid driver’s license and vehicle for local field-based travel.
