Job Description
We're seeking a Social Worker - LCSW to join our team at WellBe. As a member of our geriatric care team, you will provide compassionate care to patients in their homes, working closely with physicians, advanced practice clinicians, and other healthcare professionals. Our LCSWs play a critical role in delivering primary, urgent, and acute care, as well as counseling to patients with social and behavioral problems. Requirements
- Master’s Degree in Social Work
- 5+ years clinical social work experience with the geriatric population
- Current unrestricted LCSW license in applicable state(s)
- Ability to make psychosocial assessments and develop and implement viable care plans
- Strong computer skills, including Word, Excel, and Powerpoint
