Supervises and oversees the work activities of Case Management social work staff and develops departmental operations. May carry a caseload and provides services to patients and families.
Requirements
- Assists in selecting staff, trains/orients personnel, evaluates performance, prepares work schedules and makes recommendations for personnel actions.
- Assists in developing budgets.
- Identifies needs in program planning and facilitates program development.
- Supervises patient care, family communication, and record keeping.
- Collaborates with other hospital programs and community agencies.
- Provides direct service to patients including interviewing patients and families, preparing social histories, identifying problems, and developing treatment.
- Prepares and maintains written documentation as required by the profession and the department.
- Provides therapeutic intervention with patient and family including individual, marital, parental, family and group therapies.
- Oversees the implementation and daily operations of the lean daily management system
- Assesses and ensures staff engagement through leadership rounding and employee engagement survey results
