Licensed Practical Nurse - Home Health - Days

Provides comprehensive family-oriented nursing services to individuals in the home setting by use of the nursing process in accordance with physician orders, the established plan of care, corporate policy and procedures, and the Nurse Practice Act.

Requirements

• Participates in the development of a written plan of care
• Provides selected nursing procedures and treatments to patients
• Notifies physician and other care agencies of changes in patient’s condition and care needs
• Assists with coordination of patient care through dialogue with other disciplines involved with the patient
• Documents observations and intervention in the patient’s clinical record
• Functions as a member of an interdisciplinary team
• Fosters a climate of mutual respect and teamwork among co-workers and management
• Assist in the orientation of new staff
• Functions in a cost-effective manner
• Meets or exceeds established productivity and quality assurance standards
• Seeks consultation, guidance and direction from professional or administrative staff
• Informs manager of perceived problem areas related to patient care, corporate management, or community relations
• Keeps clinical skills current with nursing practice norms
• Exhibits a comprehensive knowledge of, commitment to and demonstrated ability in the practice of home health nursing
• Exercises sound judgment and works effectively under pressure
• Demonstrates a working knowledge of skills and services provided by each member of the multi-disciplinary team and the ability to relate these to identified patient needs
• Acquires and utilizes knowledge of community resources
• Elicits feedback from appropriate sources regarding the quality of services provided
• Demonstrates a professional attitude, sound judgment and continuity of interactions with physicians, other health care providers and community agencies
• Represents the VNA at community activities, as delegated
• Promotes a positive corporate image
