Job Description
Join Our Heart-Forward Team in Chicago! We are seeking a Pediatric Physical Therapist to provide 1:1 care in a supportive, child-centered outpatient clinic. The ideal candidate is compassionate, creative, and eager to partner with families, caregivers, and fellow clinicians.
Requirements
- Deliver Direct Care: Provide 1:1 direct patient care for full treatment sessions, focusing on play-based, evidence-driven therapy that supports children across home, school, and community settings.
- Comprehensive Assessment: Conduct comprehensive evaluations and assessments for children with developmental delays and physical conditions.
- Individualized Planning: Develop and implement individualized treatment plans tailored to each child's goals.
- Collaboration: Collaborate closely with families, caregivers, and interdisciplinary providers.
- Clinical Excellence: Maintain detailed, timely clinical documentation and progress notes (EMR: EnableDoc).
