Value Based Care Manager - Full Time - Wolf River

We are seeking a Value-Based Care Manager to coordinate and manage the care of oncology patients participating in value-based care programs. The role serves as a central point of contact for patients and caregivers, ensuring care coordination, reducing duplication of services, and supporting positive clinical and cost-effective outcomes.

Requirements

• Bachelor's degree in Nursing
• Minimum of three (3) years of nursing experience
• Current RN license to practice in the state where work is performed
• Strong knowledge of healthcare management resources and care coordination services
• Excellent verbal and written communication skills with strong problem-solving and conflict resolution abilities
• Ability to communicate effectively with patients, families, physicians, and staff at all levels
• Proficiency in Microsoft applications (Word, Excel) and understanding of patient privacy and security requirements
• Ability to work independently, exercise sound judgment, and manage multiple tasks and projects effectively
• Strong organizational skills with the ability to maintain control of workflow in a fast-paced environment
