Job Description
Community Health Worker (CHW) responsible for encouraging patient and provider shared decision-making, promoting healthy patient behaviors, improving medication adherence, and linking clinical care with community-based social supports.
Requirements
- Act as a liaison between patient and care team
- Work cooperatively with other staff in coordinating patient care
- Meet regularly with the care team to plan care and discuss cases
- Mentor and coach patient, making connections to community supports
- Identify and provide linkages for services in community partner agencies
- Treats guests, patients, physicians, and other employees with care, courtesy, and respect
- Works cooperatively within own department and other areas
- Keeps appropriate people informed
- Speaks and writes clearly, concisely, and appropriately
- Listens carefully
- Communicates tactfully
