Requirements
- Doctor of Medicine or Doctor of Osteopathic Medicine with relevant Board Certification or Board eligibility by the American Board of Psychiatry and Neurology
- Licensed physician in the State of Connecticut
- Active DEA controlled substance registration and an active Connecticut controlled substance registration
