World Relief is seeking a Community Health Worker to serve in our WR Modesto Office. The Community Health Worker (CHW) plays a vital role in supporting Californiaâs Medi-Cal transformation under CalAIM by providing relationship-based, whole-person support grounded in lived experience.

- Able to affirm and/or acknowledge World Relief's Core Beliefs, Statement of Faith, Christian Identity and National Association of Evangelicals' For the Health of The Nation document
- Bachelorâs degree in Social Work, Psychology, Nursing, or a related field; OR Associateâs degree in Social Work, Psychology, or related field with at least 2 years of mental health or social services experience; OR equivalent combination of training and professional experience.
- Minimum of 2 years of experience in case management, care coordination, or related social services role.
- Certified Community Health Worker (or willingness to obtain certification within a designated timeframe).
- Strong cultural competency and sensitivity to the needs of diverse populations.
- Proficient verbal and written English communication skills.
- Bilingual or multilingual in a language commonly spoken in the client community (Dari, Pashto).
- Knowledge of HIPAA and strong commitment to maintaining client privacy and data security.
- A valid driverâs license, current auto insurance, reliable transportation, and a satisfactory Motor Vehicle Record (MVR) are required.
