Certified Diabetes Educator

We are seeking a Certified Diabetes Educator to develop, manage, and coordinate the outpatient diabetes program at Thomas Memorial Hospital. The incumbent will be responsible for outreach to physician practices, patient assessment, education, and evaluation of patient outcomes. They will also participate in community outreach and educational support, and advocate on behalf of patients and families.

Requirements

• Certified Diabetes Care and Education Specialist (CDCES) through the Certification Board for Diabetes Care and Education
• One year experience in health and fitness
• ASC Clinical Exercise Physiology Certified (preferred)
