Coordinator

Assist patients through the healthcare system by acting as a patient advocate and navigator, facilitating health and disease education, and promoting clear communication amongst care teams.

Requirements

• Participate in Patient Centered Medical Home team meetings and quality improvement initiatives
• Facilitate health and disease patient education
• Support patients' self-management of disease and behavior modification interventions
• Coordinate continuity of patients and families following hospital admission, discharge, and ED visits
• Conduct comprehensive, preventive screening for patients
• Promote clear communication amongst care teams
• Facilitate patient medication management
• Participate on a team for data collection, health outcomes reporting, clinical audits, and programmatic evaluation
• Evaluate clinical care, utilization of resources, and development of new clinical tools, forms, and procedures
