Job Description
The Clinical Team Leader - North will supervise a team of START Therapeutic Coaches, providing in-home coaching services across assigned regions. The Team Lead will hire, train, and support the Coaches in all aspects of their role, within the START model.
Requirements
- Provides oversight and supervision of the START Therapeutic Coaches
- Assigns cases to Therapeutic Coaches and assists with scheduling
- Assists Therapeutic Coaches with identifying strategies and interventions
- Monitors completion of necessary trainings for staff
- Collaborates with Coordinators and Clinical Director in planning and scheduling
- Communicates with caregivers and circles of support
- Ensures accurate documentation of coaching activities
- Participates in recurring consultation and staff meetings
- Supports regular outreach with community partners
- Facilitates discharge planning meetings
- Serves as Administrator-on-Call on a regular basis
- Completes and maintains necessary trainings
