Social Worker (MSW)

YAI has an opening for a full-time Social Worker within our Kew Gardens site in Queens. The position provides exposure and experience working within the Queens Crisis Intervention Team and other professionals at YAI.

Requirements

• Provide in person intake/home visits to assess service needs and provide needed services.
• Treats people we support with disabilities related to the areas identified in intake, such as, case management, behavior management, and crisis counseling in the home.
• Facilitate Parent Support Groups (In person).
• Participate in interdisciplinary care planning for people we support and their families.
• Participate in utilization/quality assurance reviews.
• Provide outreach presentations on YAI Crisis Intervention and other related programs within and outside of the agency.
• Perform responsibilities as they currently exist or may change in meeting the needs of the department.
