Case Manager

Youth Advocate Programs, Inc. is seeking a full time Case Manager to provide comprehensive case management services to individuals and families in need.

Requirements

• Master’s degree preferred; Bachelor’s degree in social work, psychology, or a related field.
• Experience working in a community-based agency program.
• Proven experience in case management or similar role
• Strong knowledge of social issues and community resources
• Excellent interpersonal and communication skills
• Organized and has good time management skills
• Proficient in Microsoft Office 365
• Knowledge of Electronic Health Record Systems (EHR) is a plus.
• Bilingual (Spanish) is a plus
• Reliable transportation, valid driver's license, and current auto insurance coverage is required.
