Wraparound Facilitator

Wraparound Facilitator will work with families of children with serious emotional disturbances by facilitating a planning process that is family centered, community-oriented, strength-based, and highly individualized.

Requirements

• Bachelor's Degree in social work, human services field or related
• Proficient in Microsoft 365
• Experience using an electronic health record system (EHR) is a plus
• Strong communication and organizational skills
• Bilingual (Spanish Speaking) is a plus
• Reliable transportation, valid driver's license, and current auto insurance coverage
