Job Description
We are looking for a motivated and driven Children's Mental Health Practitioner to work with clients and their families to improve their mental health and wellbeing.
Requirements
- Assess client and family needs by collecting, evaluating, and documenting information
- Develop service plans in cooperation with the family and other providers
- Coordinate and make referrals for needed services
- Provide face to face support services to clients
- Maintain knowledge of city and/or county mental health, medical, financial, housing and other human and family services agencies
