21st Century Home Health Services (HHS) is seeking a full-time Licensed Vocational Nurse (LVN) to provide comprehensive in-home care for over 1,700 patients in Alameda County and San Francisco. The role focuses on promoting wellness and preventing rehospitalization, with a focus on wound care, medication education, and case management. Requirements
- Current and unrestricted CA LVN license
- Current BLS/CPR Certification
- Valid CA Driver's License
- High level of clinical knowledge in home health care is highly preferred
