Licensed Vocational Nurse (LVN) - Home Health

21st Century Home Health Services (HHS) is seeking a Licensed Vocational Nurse (LVN) to provide care to patients in their homes, specifically in San Francisco, Santa Clara, Walnut Creek, and Alameda counties. They are a large, growing agency known for exceptional patient outcomes and employee satisfaction. The role involves patient care, case management, and collaboration with interdisciplinary teams.

Requirements

• Current and unrestricted CA LVN license
• Current BLS/CPR Certification
• Valid CA Driver's License
• High level of clinical knowledge in home health care (preferred)
• Experience working with high acuity patients (a plus)
• Experience with an interdisciplinary team (highly desired)
