Job Description
21st Century Home Health Services is seeking a Registered Nurse (RN) Home Health Case Manager to join our team. As a RN Home Health Case Manager, you will create, implement and execute the overall plan of care to promote wellness and prevent rehospitalization for patients in their homes.
Requirements
- Current and unrestricted CA RN license
- Current BLS/CPR Certification
- Valid CA Driver's License
- High level of clinical knowledge in home health care is highly preferred
- Experience with high acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired
- Case Management fundamentals is highly desired
