Job Description
We are seeking a Registered Nurse (RN) Home Health Case Manager to join our team. As a RN Home Health Case Manager, you will create, implement and execute the overall plan of care to promote wellness and prevent rehospitalization for patients in their homes. You will utilize your outstanding clinical skills and work with patients and family one on one to achieve improvement and/or stability.
Requirements
- Current and unrestricted CA RN license
- Current BLS/CPR Certification
- Valid CA Driver's License
- High level of clinical knowledge in home health care is highly preferred
- Experience with high acuity patients is a plus
- Experience working with an interdisciplinary team is highly desired
- Case Management fundamentals is highly desired
