A COMMUNITY OF FRIENDS is a nonprofit organization that manages residential properties and delivers supportive housing and case management services to formerly homeless individuals and families, ensuring maintenance, tenant support, and comprehensive care.
This role involves providing individualized case management to formerly homeless individuals and families, utilizing a Motivational Interviewing, Critical Time Intervention, and Stages of Change framework. The CM-I will perform various tasks such as intake, assessment, goal setting, referrals, and crisis management, documenting all interactions in the HMIS database. They will also be responsible for reporting abuse as a mandated reporter.
