Job Description
VIP Community Services is seeking a dedicated Case Manager to provide case management services to residents in homeless shelters. The role involves coordinating care for individuals with substance use and mental health challenges, including medical, psychosocial, and advocacy services. This position supports the delivery of services according to agency policies and DHS regulations.
Requirements
- Provide case management to clients in accordance with policy, procedure, and regulatory protocols.
- Conduct initial assessments of referrals and develop care plans.
- Follow-up with clients weekly, documenting progress, and meeting with case reviews.
- Coordinate with housing specialists to facilitate housing placement.
- Escort clients to appointments and provide data collection.
- Advocate on client's behalf regarding Foster Care / Welfare Cases.
- Maintain a positive image of the organization in the community.
