The Case Manager I (CM-I) will provide individualized case management to formerly homeless individuals and families, utilizing a motivational interviewing, critical time intervention, and stages of change framework. This role focuses on comprehensive services, including intake, assessment, goal setting, and monitoring, and involves documentation in the HMIS database. The CM-I will work within a case management system and assist with crisis intervention and reporting. This position contributes to reducing homelessness.
