This position offers Intensive Case Management Services (ICMS) to formerly homeless individuals and families with chronic medical conditions and co-occurring disorders. The CM-I will provide individualized case management, motivational interviewing, and trauma-informed care, collaborating with both tenants and external organizations to achieve their goals and promote stability. This role supports individuals in maintaining stable housing and accessing resources.
