ICMS Case Manager 1

This ICMS Case Manager position supports formerly homeless individuals and families with chronic medical conditions and co-occurring disorders in Pacoima, CA. The CM-I (ICMS) will provide individual case management including intake, assessment, goal setting, and referral services. Collaboration with the Tenant Services Supervisor and support of a comprehensive case management framework based on MI, CTI, and SoC principles is essential. The role requires documentation, reporting, and coordinating with various agencies and stakeholders.

Requirements

• Basic Qualifications: Bachelor’s degree from an accredited college or university
• Two (2) years case management/client advocacy services with people who have been homeless and/or have a mental illness
• One (1) year experience utilizing a client tracking database system
• One (1) year experience utilizing evidence based practices in a social services field
• Demonstrated experience in developing and facilitating providing life skills groups
• Proficiency with Microsoft software programs (Word, Excel, PowerPoint, Outlook)
• Communicate effectively both in verbal and written formats
• Sensitivity and appreciation of diverse tenant populations
• Ability to provide non-judgmental support and direction
• Proficiency with the CHAMP and HMIS database systems
• Ability to provide crisis management services
• Valid California driver’s license
• Access to a personal vehicle
