This ICMS Case Manager position supports formerly homeless individuals and families with chronic medical conditions and co-occurring disorders in Pacoima, CA. The CM-I (ICMS) will provide individual case management including intake, assessment, goal setting, and referral services. Collaboration with the Tenant Services Supervisor and support of a comprehensive case management framework based on MI, CTI, and SoC principles is essential.
